Job: CDTC CARE COORDINATOR- CHILDRENS DIAGNOSTIC & TREATMENT CENTER

FULL-TIME - MONDAY - FRIDAY - DAYS

Bachelors degree in Social Work or related field. Two years of experience working with infants and toddlers who have special health care needs. Must be able to conduct comprehensive assessments and provide support services and education to high risk families. Must have good organizational and communication skills. Bilingual (Spanish/Creole) preferred.
